To enroll your child(ren) in the Religious Education Program you will need to DOWNLOAD and complete both of the following forms – 1) Religious Education Registration Form; and 2) Diocese Harassment Policy Verification Form – and mail along with your payment to Our Lady of Victory Church, Attn: Religious Education, 1559 Roxbury Road, Columbus, Ohio, 43212. Check must accompany your forms in order to complete your registration – DO NOT email these forms.
